The Gotfried PH Nail™ is designed for the management and stabilization of trochanteric, subtrochanteric and base of neck hip fractures.

Based on more than 20 years of clinical observation and surgical developments, The Gotfried PH Nail™ was designed following a rigorous analysis of currently available Intra Medullary Hip fracture devices, identifying their weaknesses and drawbacks and finding ground breaking solutions to these. Physiologic and Biomechanical improvements have been applied which have been shown to enhance surgical outcomes. The implant design is based on the STABLE DYNAMIC™ principles. Each stage in the surgical procedure for pertrochanteric, subtrochanteric and base of neck fractures was addressed, aiming at reducing operative trauma to patients whose general condition is frequently compromised by severe concomitant medical or surgical conditions, and aiming at ensuring that walking ability is maintained.

 


The PH Nail System is the advanced, second generation of implementing the principles of STABLE DYNAMIC TM fixation. The first generation, the PC.C.P (Percutaneous compression plating) has demonstrtated a significant reduction in both operative and postoperative complications including “cut outs” and fracture collapse. The PH Nail™ represents a minimally invasive method of Intra Medullary Trochanteric fracture stabilization and fixation, providing enhanced rotational stability and bone sparing. Furthermore, the "Blocking effect" often seen with currently available Intra Medullary devices, which can prevent or delay healing, a critical impediment with other devices, had to be and has been overcome with this new design.

Immediate, postoperative weight bearing may be permitted.



The PH nail technique is designed around five fundamental principles:
1. Closed Fracture Reduction to a 135° shaft/neck angle.
2. Minimally Invasive Surgery.
3. The Provision of Rotational Stability.
4. Prevention of the blocking effect, thereby facilitating Controlled Fracture Impaction.
5. Securing the barrel to the nail thus preventing the Z effect.

Dedicated surgical instruments made of special implant quality stainless steel as well as carbon composite are used.

Nail:
Available in two lengths: long nail – 204mm and short nail – 175mm.
The proximal end of the nail is connected to the Introducer / Targeting Device. Two oblique screws (Long Screws) are screwed into the femoral neck through two nail holes in the proximal section of the nail and one short shaft screw will anchor the nail distally to the femoral shaft.

Long Screw:
Two screws with a choice of 10mm increments from 90mm to 130mm are used with each procedure. The long screws are telescopic with a sleeve section (which locks into the nail holes) and a self-tapping threaded section. The self-tapping threaded section is advanced and screwed into the femoral head. The screw is open when completely telescoping out and closed when completely telescoping in.

Short Screw:
One self-tapping screw with a choice of 3mm increments from 31mm to 46mm, is used with each procedure. The threaded recess within the screw head is used to attach the screw to the screwdriver.

The Gotfried PH Nail™ for trochanteric and subtrochanteric femoral fractures is intended as a means of bone stabilization in the management of fractures. The Gotfried PH Nail™ is designed specifically for the fixation of trochanteric, base of neck and subtrochanteric fractures of the proximal femur. The product is intended for professional use only. Surgeons who supervise the use of the product must have full awareness of orthopaedic fixation as well as adequate PH nail training and understanding of the philosophy of the product and technique.

 

 
   
   
   
   
   
   
   
 
 
 

Use of the Gotfried PH Nail™ is contraindicated in the following:
1. Sub capital femoral fracture.
2. Active infection.
3. Acute medical conditions including: impaired blood supply, pulmonary insufficiency (i.e. ARDS, fat embolism) or latent infection.
4. Foreign body sensitivity. Where material sensitivity is suspected, tests should be made prior to implantation.
5. Insufficient bone quantity or quality.
6. Pathological fractures are recommended to be treated with additional means like bone cement.
